If you have a child that will be entering Kindergarten this fall, now is the time to be thinking ahead and making sure that all of their health requirements are being met so that you don't have to try and rush to get everything done during August. Your child can have their kindergarten booster shots any time after they have reached the age of four. If you do not have a regular doctor to provide these, Warren County Public Health will do immunizations if you give them a call and make an appointment with them. Their number is 515-961-1074. If your child is a preschool student here you can call me to verify if your child needs vaccinations or not. While you are at the doctor, ask them if your child has had a lead screening done. The state does require a lead screening be done on all students, and the county does audit that requirement. Dental Screenings are required for students in Kindergarten. When you see your dentist please ask him to fill out a screening form for you and bring that to school. If you do not have a dentist please give me a call and I can do the screening for you. Lastly vision cards were sent out in your kindergarten round up packets. There is not a requirement at this time that your child have their eyes screened, but if you do, please have them fill out the green card and send it back to school. All of these items can be faxed to school if that is convenient for you. Our fax number is 641-764-2100, please address it to me...Alene, or the Nurse, so that is directed to me when it arrives. If you have questions about any or all of these items please feel free to call me during the day from 8:00 til 1:45. If you call after that time leave a message and I will get back to you as soon as possible. Thanks, Alene Kirvin, R.N. Martensdale-St. Marys CSD alene_kirvin@m-stmarys.k12.ia.us |
