New Student Registration

All new students entering Martensdale-St. Marys Community School District must start
their registration process in the Central Office.

All new students entering Martensdale-St.Marys Community School District must provide the proper documentation to establish residency. The documents provided must always state the student’s physical address. A post office box is not an acceptable address for the purpose of determining residency.

You must have proof of residency in order to enroll your child. If you do not know whether your residence is within the Martensdale-St.Marys Community School District, contact Jill Gavin, Business Manager at 641.764.2608.

New Students Only:
*New Students (7-8) will need to meet with the Guidance Counselor to set up your class schedule. Request of Records from the previous school will also need to be made.

*New Students (9-12) Records from the previous school must be available before your class schedule can be set up.

Please attend registration at the school on August 13, 2019 from 12:00 p.m. - 8:00 p.m.
You will need to complete the following forms:  

  1. New Student Registration Information
  2. Student Health Update
  3. Athletic Physical (7-12)
  4. Student Permission Information
  5. Dental Screening (kindergarten, 9th grade and all out-of-district transfer students)
  6. Athletic Medical & Transportation Eligibility (7-12)
  7. Student Driving Permit Form (7-12)
  8. HEADS UP: Concussion in High School Sports
  9. Before and After Form
  10. Home Language Survey

You will need your child’s up to date immunization record, dental screening, lead screening (preschool and kindergarten only) and proof of birth; i.e., a birth certificate.

Pay school fees or complete a fee waiver and free/reduced lunch application.

Bring any special needs documents if applicable. (Example: Individualized Education Plan / IEP, 504 Plan, Gifted and Talented / Enrichment Plan, Individualized Health Plan (IHP), etc.)

Name, address and phone number of your child(ren)’s previous school to request records.

If you are not able to register on August 13, please contact Karen Tomlinson or 641-764-2466 to set up an appointment.  You must have all of the documentation with you before we can enroll your child(ren) in the Martensdale-St. Marys Community School District. 

Competent Private Instruction (Home Schooling)

Please return all forms to the appropriate offices and pay all registration fees online by August 23, 2019.  Registration is not complete until registration fees have been paid.  If payment arrangements are not made by August 23, 2019 these fees will be sent to the school’s collection agency.

RevTrak is a secure site and does not remember any of your personal information.  You can use a checking account, debit card, or credit card to pay fees online.  If you are using RevTrak you will be able to pay registration fees and lunch at one time.  Please go to and click on Blue Devils Web Store.
Paying online is the preferred option. If you prefer to pay by check, you will need to proceed to the appropriate office. Separate checks will need to be written out for Registration, Hot Lunch and Athletic passes. The money goes to different accounts and we would like to keep them separate.
