Iowa Immunization Requirements

Preschool

Physical Form – all preschool students must turn in a physical form signed by a medical provider.

Immunization Certificates – one of the following must be turned in:

• Certificate of Immunization

• Medical Exemption Form

• Religious Exemption Form

Kindergarten

Please have these forms filled out by medical/dental providers and turned in on the first day of school.

Dental Screening

Vision Screening

Immunization Certificates – one of the following must be turned in:

• Certificate of Immunization

• Medical Exemption Form

• Religious Exemption Form

3rd Grade

Please have these forms filled out by medical/dental providers and turned in on the first day of school.

Vision Screening

7th Grade

T-Dap and Meningococcal are needed and HPV is recommended.

Immunization Certificates – one of the following must be turned in:

• Certificate of Immunization

• Medical Exemption Form

• Religious Exemption Form

9th Grade

Please have these forms filled out by medical/dental providers and turned in on the first day of school or before.

Dental Screening

12th Grade

Meningococcal vaccine, recommend Meningitis B
